Definition: A biofilm is a structured community of microorganisms, often encased in a self-produced extracellular polymeric substance (EPS) matrix, that adheres to a surface. These microbial collectives can consist of bacteria, fungi, and other microbes living synergistically.

Biofilms initiate when planktonic (free-floating) microorganisms attach to a surface, which can be biological (e.g., human tissue, teeth) or inert (e.g., medical devices, pipes). Once attached, these microbes proliferate and secrete a complex matrix of polysaccharides, proteins, and nucleic acids, known as the extracellular polymeric substance (EPS). This EPS matrix acts as a protective shield, embedding the microbial community and facilitating cell-to-cell communication through a process called quorum sensing, allowing for coordinated behavior and enhanced survival.

The significance of biofilms in public health is profound, as they are implicated in a vast array of persistent and difficult-to-treat infections. Their protective matrix makes the embedded microbes highly resistant to antibiotics, disinfectants, and the host immune system, often requiring higher concentrations of antimicrobial agents or physical removal. Biofilms are a leading cause of healthcare-associated infections (HAIs), particularly on indwelling medical devices like catheters, prosthetic joints, and dental implants. They also contribute to chronic wound infections, cystic fibrosis lung infections, and contamination of water distribution systems (e.g., Legionella). The recalcitrance of biofilm infections leads to prolonged illness, increased healthcare costs, and contributes significantly to the global challenge of antimicrobial resistance.

Key Context:

  • Antimicrobial Resistance: Biofilms significantly enhance microbial resistance to antibiotics and disinfectants, complicating treatment.
  • Healthcare-Associated Infections (HAIs): A major cause of infections related to medical devices and hospital environments.
  • Extracellular Polymeric Substance (EPS): The protective matrix that encases biofilm microbes, providing structural integrity and resistance.
